What are the check off list for filling up canteen smart card application form
  • Check that all the entries be made in capital letter with black pen.
  • Photographs pasted on the form to be in civil dress. It should be of high quality and not computer printout.
  • Compare the correctness of your particulars with discharge book.
  • Ensure that a separate application be filled for reapplying and fresh form case.
  • Application form should be supported by a copy of ppo and the form should be countersigned by station hq / zsb whichever is close to the esm. In case of non -pensioner it should be supported by record / discharge book and countersigned by station hq/ zsb whichever is closer to the esm. In case of war widow it should be supported by copy of ppo and to be signed by station hq/ zsb whichever is closer to the esm. The countersigning authority should be in the rank of colonel / director or equivalent. In case of world war I and ii veterans and widow the form should be supported by copy old record / discharge book alongwith proof of marriage.
  • Tha validity of the card will be ten years. Beneficiaries are requested to apply for fresh card at least three months in advance.
What are the eligibility criterion and check off list for issuing / renewal of dependent pass
  • The following are eligible:-
    • (a) Spouse and dependent children of esm.
  • Validity of dependents card
    • (a) in case of war / pensioner’s widow, the identity cards will of permanent nature and remain valid till she gets remarried. It may be renewed every five years.
    • (b)the identity cards in respect of spouse of esm will also of permanent nature and would necessitate change only on change of status (if divorced), to be renewed every five years.
    • (c) dependents cards issued to the children of esm will be issued initially for a period of five years and renewed thereafter for another five years depending on following:-
      • Sons - attaining the age of 25 years or on ceasing to be dependents whichever is earlier or unemployed due to disabled life time.
      • Daughters - till married or unemployed due to disability life time.
      • Permanent disabled children - life time.
    • (d) Separate application form to be filled for each dependent.
    • (e) following docs are to be produced by esm for issue of dependent cards:-
      • Application with photographs pasted.
      • Registration form.
      • Copy of discharge book.
      • Copy of ppo
      • Copy of birth certificate.
      • Copy of aadhar card.
    • (f) issue of duplicate dependents cards:-
      • In case of loss, esm should approach zswo along with copy of fir and application containing the facts leading to the loss of cards.
What is part ii order and why is it required
  1. Any amendments required to be done in the discharge book, a part ii order will be issued by navpen. Procedure for updation of discharge book is as appended in succeeding paragraphs:-
    • (a) for change of permanent address- the esm will submit an application to navpen through concerned zsb seeking to record amendment in the permanent address in discharge book. The zsb will forward the application to navpen. The navpen will issue a part ii order and forwarded it to concerned zsb and also send a copy of that order on the new and old address of applicant.
      • Original discharge book need not to be sent along with application.
      • This application should be attached with certified copy of address proof.
  2. Procedure for recording change / inclusion of date of birth and change of name
    • The esm will submit an application to navpen through concerned zsb seeking to record the change of name or date of birth. The application should be attached with supporting documents like birth certificate/ school leaving certificate or affidavit sworn before a magistrate or notary stating state / birth place etc.
  3. For change of name - publish change of name in gazette of india part iv and submit duly attested printed matter. Advertise the change of name in daily newspaper. Submit an affidavit duly attested by first class magistrate / notary / oath commissioner.
  4. Endorsement of marriage/ marriage declaration post retirement
    • The esm / veernari is required to submit an application along with requisite documentry proof (attested copies of marriage certificate, birth certificate / matriculation certificate and aadhar card / pan card of wife) to navpen duly attested by zsb. Original court affidavit having detail of esm and spouse seeking marriage declaration be also sent along with application.
  5. Endorsement of death / divorce of first wife and re-marriage with 2nd wife.
    • The esm is required to submit an application along with requisite documentary proof to navpen through concerned zsb.
  6. Documentary proof- original divorce deed or attested copy of death certificate of first wife as applicable. Attested copies of marriage certificate, birth certificate / matriculation certificate, aadhar or pan card of wife, original court affidavit having detail of esm & 2nd wife seeking marriage declaration.
  7. Endorsement of name of new born child / adopted child.
    • The esm / veernari is required to submit an application along with requisite documentary proof to navpen duly attested by zsb.
  8. Documentary proof- attested copies of birth certificate, adoption deed certificate for adopted son or daughter, original court affidavit declaring birth of child attested by 1st class magistrate.
  9. Endorsement of nok name – dependent (divorcee / widowed/ unmarried) daughter or dependent son.
    • The esm / veernari is required to submit an application along with requisite documentary proof to navpen duly attested by zsb.
  10. Documentary proof in case of dependent daughter - original divorce deed with husband or attested copy of death certificate of husband whichever is earlier. Attested copies of marriage certificate, birth certificate / matriculation certificate, aadhar or pan card of daughter & original court affidavit having detail of daughter’s date of birth, marital status and source of income, attested copy of income certificate.
  11. Documentary proof in case of dependent son - attested copy of birth certificate, or xth certificate alongwith attested copy of aadhar acrd / pan card. Original court affidavit having detail of son’s date of birth, marital status and source of income & attested copy of income certificate.
  12. Change of name of esm / dependent.
    • The esm / veernari is required to submit an application alongwith requisite documentary proof to navpen through zsb.
  13. Documentary proof in case of dependent daughter-
    • duly attested printed copy of publication of change of name in the gazette of india / state gazette.
    • advertisement clipping of change of name in any one daily nationalized newspaper.
    • original court affidavit declaring old and new name along with detail of date of birth duly attested by 1st class magistrate.
    • Attested copies of two photo id proof with full and correct name mentioned along with date of birth
  14. Change of date of birth of esm dependents.
    • The esm / veernari is required to submit an application along with requisite documentary proof to navpen through zsb.
  15. Documentary proof in case of dependent daughter
    • Attested copies of birth / matriculation certificate and aadhar / pan card of dependents.
    • Original court affidavit declaring date of birth of child duly attested by 1st class magistrate/ notary
  16. Disability certificate for differently abled son/ daughter.
    • The esm / veernari is required to submit an application along with requisite documentary proof to navpen through zsb.
  17. Documentary proof
    • disability certificate (afmsf-hd -1) for handicapped / mentally retarded child issued by any service classified specialist of the rank of brigadier or above or by a civil surgeon clearly stating that the disabled person is unable to earn a living for his own.
    • original court affidavit having details of child’s date of and disability details.
  18. Pay fixation cases.
    • Application with appendix ‘b’ of ihq mod(n)/ desa letter dx/vsf/165/15 dated 18 feb 15 is to be forwarded by the concerned office where the ex sailor is presently employed.
    • To npo / sailors de-mobalisation section along with. The esm / veernari is required to submit an application along with requisite documentary proof to navpen through zsb.
  19. Enrolment of esm in dsc as jco / ors. The esm / serving sailor are required to submit an application along with documentary proof to navpen. Ex-serviceman should apply within two years from date of discharge whereas serving sailor should apply atleast six months prior to their release for enrolment in dsc. Desa will forward such application to ins agrani post scrutiny. Post verification ins agrani forward the application to dsc records at kannur kerala.
  20. Eligibility criterion for enrolment into dsc
    • Age should not be more than 50 years.
    • medical cat at the time of release should be s1a1.
    • min education qualification of applicant should be ssc/ matriculation.
    • Applicant should be naval pensioner
    • Rank should be equivalent to cpo and above for dsc jco and should be po & below for dsc as or commission.
    • Assessment for the last five year should be very good & superior.
    • No punishment / red ink entries in service record book for dsc as jco commission applicant.
    • Not more than one punishment / red entries in the last five years for dsc as or commission applicant.
  21. Documentary proof
    • Duly filled application form
    • Attested copies of pages of sd (page 1-20)
    • Mark sheet of ssc / matriculation.
  22. Issue of service particulars to the widow of ex sailor.
    • The veernari is required to submit an application along with requisite documentary proof to navpen duly attested by their last unit / crso / concerned zsb for issuance of service particulars.

    • Note:The service particulars in lieu of discharge book is issued only one time.
  24. Issue of sea service extract. The esm/ serving sailor is required to submit an application along with requisite documentary proof to navpen duly attested by last unit / zsb .

    • Photocopy of service certificates in-271(for ex sailors only)
  26. Verification and countersign of dependents pass application form.
    • As per ksb policy no 061/policy/genpol/vol-1 dated 03 aug 18, all naval esm are requested to approach to their concerned zsb for verification and countersigning of dependents id card form.
  27. Verification and countersigne of canteen smart card application form.
    • As per canteen policy no 95350/q/ddgcs/policy dated 11 apr 18, all naval esm are requested to approach to their concerned zsb for verification and countersigning of canteen smart card form.
  28. Issueance of performa ‘a’ and character certificate.
    • As per existing policy there is no provision for the issuance of these certificates from navpen. Also all the information in proforma ‘a’ certificate and character certificate is available in the discharge book and other certificates issued to the sailor during the time of release.
  29. Dual family pension. Spouse / nok of service pensioner authorised to draw two family pension (military & civil) wef 24 sep 12 or thereafter. (auth: goi/mod letter no 01(05)/2010-d(pen(policy) dated 17 jan 13)
  30. Transfer of pension. Open a pension account in the bank from which paension to be drawn.
    • Submit an application to the existing bank for transfer of pension indicating s/b account no and full address of the new bank
    • Intimate change of pension account to navpen and pcda for issue of corrigendum ppo
  31. Loss of ppo.Bank loosing ppo to furnish a loss certificate to concerned psa (pcda (p)/pcda (navy)
    • If ppo is lost by idividual, apply to pcda(navy) allahabad for issuance of duplicate ppo alongwith mro & fir.
  32. Family pension. Approach the pension disbursing bank with original copies of ppo, death certificate of veteran & pension book which has joint photo.
  33. Aadhar registration & jeevan praman. To facilitate online rendering of annual life certificate by all pensioner, govt of india has launced digital life certificate (jeevan praman) scheme. It functions through an online portal (http://jeevanpraman.Gov.In) which allows pensioners to link their aadhar card with pension bank account and annual life certificate. The pensioner are initially required to to register themselves with jeevan praman portal using aadhar detail, pension detail and biometrics. To record biometric for initial registration, the pensioners may either contact their pension bankor nearest citizen service centre. After the initial registration, pensioners are required to sunmit their biometric every year through the life praman portal, which in turn will generate the digital life certificate and send it to the concerned banks.
  34. Note: pensioner can buy a biometric recorder and install the jeevan praman apps in their pc / laptop / smartphones and render the annual life certificate without visiting bank.
  35. Action of nok incase of death of veteran.
    • informed concerned crso
    • surrender esm cards and seek widow I-card from zsb
    • initiate action for change of canteen and echs smart card
    • maintain a file whioch contain following documents.
      • Original ppo and & all corr. Ppo
      • Annexure 4
      • Death certificate
      • All correspondence with ihq, bank echs polyclinic, csd & zsb.
      • Proof of dob
      • Address proof
      • Original copy of citation (for gallantry awardee).
      • Pension disbursement details
      • Contact detail of concerned crso and desa
  36. Esm I-card. Approach to zsb with following docs.
    • Ppo / proof of gratuity payment
    • Discharge book / service certificate
    • Three copies of stamp size photographs.
  37. Widow I-card. Approach zsb with following docs.
    • Deceased veteran zsb I-card.
    • Copy of death certificate.
    • Copy of ppo
    • Address proof
    • Three stamp size photographs.
  38. Grants to retired naval pensioners and their families.
  39. Merit scholorship.
    • Scholorship (fresh or renewal) for 10+2 education admissible for first two children only to pursue graduation or post-graduation course as per laid down eligibility criteria. Application to be forwarded to secy. Inba latest by 01 nov of every year through zsb (for esm) and crsos (for widows). Scholarship not availing during any year for any reason can’t be claimed in subsequent years.
  40. Special scholorship scheme (sss).
    • Application for wards of naval personnel who dies in harness. Re-imbursement of actual expenditure, subject to lay down uppen ceiling, provided for education from play school to post graduation and professional courses. Application form along with attested originals receipts / bills 9only for 10+2 courses) counter signed by principal, be forwarded to inba latest by 31 jul every year.
  41. Scholorship for orphaned children.
    • Scholorship for orphaned children, irrespective whether parents died in serviec or after retirement, introduced wef academic years 2015-16. Rates and eligibility same for sss as mentioned above.
  42. Scholorship for children of widows of naval personnel.
    • Children of naval personnel who dies after retirement eligible from class I to xii, graduation & pg, subject to laid down upper ceiling. Child should pass all the examination. No % criteria laid down. To be applied yearly on prescribed form to the hony secy, nwwa, a block ihq mod(navy) new delhi 110011.
  43. Scholorship for special / disabled children.
    • Admissible for first two children only between the age of 3-25 years on production of disability certificate from competent authority. Application to be forwarded to inba through zsb by 31 apr of every year.
  44. Re-imbrushment of coaching fees.
    • 33& of the total coaching fees re-embrused for first two wards of serving / retired naval personnel and widow securing admission in iits/nits/iis(banglore)/ism(dhanbad) and mbbs. Application to be forwarded to inba through zsb within two months of admission.
  45. Assistance for self employment.
    • An online financial assistance of rs. 1,00,000/- provided to widow facing acute financial distress for stating self help economic venture. Application form to be forwarded to inba through respective crsos.
  46. Grant on death of veteran.
    • A one time grant is provided to the nok of officers and sailors on demise of naval pensioner. Application duly signed by nok to be forwarded to inba through respective crsos with following docs.
      • Attested copy of death certificate.
      • Attested copy of ppo.
      • Attested copy of discharge certificate.
      • Leaf of a self cancelled cheque showing name, ifsc code and account number clearly.
  47. Vocation courses for widows.
    • Widow of naval personnel are re-imbrussed fees up to 50,000/- for completing vocational course like b.Ed , computer courses , management courses etc. Application form intimating the course undertaken to be forwarded to secy inba through respective crsos.
  48. Daughter marriage grant to widow.
    • Provided to widow of naval personnel for marriage of their first two daughter only. Application form to be forwarded to inba two month prior to date of marriage through crsos.
  49. Assistance to family members of unmarried officers / sailors who dies in harness.
    • Following welfare scheme available for the family of unmarried officers and sailors.
      • Onetime grant for marriage of one sister only.
      • Re-imbrushment of fees for vocational courses upto rs- 50,000/- for mother 9only on case to case basis.
      • Self help economics venture upto one lakhs to mother (only on case to case basis.)
      • Special scholarship scheme (sss) extended for two unmarried sisters wef academic year 2015-16.
